The Licensed Vocational Nurse (LVN) provides intermittent Skilled Nursing services to patients in the home setting according to the patient's Plan of Care appropriate to the age of the patient served.
Additional Duties:
Perform a variety of patient care activities under the supervision of a Registered Nurse.

Provide patient care services

Act as the patient's advocate and as such as liaison to assist in communicating the patient's needs to the multidisciplinary team.

Participate in professional activities

Comply with HIPAA privacy regulations and maintains confidentiality at all times

Participate in Performance Improvement Activities Required, ability to read, write and communicate effectively in English
Required, evidence of the practice of a high level of confidentiality.
Required, effective communication skills
Required, personal vehicle with current liability insurance
Required, current and valid TX drivers license
Required, able to make sound nursing judgments
Required, computer, internet access and email capabilities from home or other off-site location
Required, High School Graduate or equivalent
Required, completed a course of study from an accredited school
Training and Experience
Required, two years of prior nursing experience
Required, licensed by the State Board of Nursing
Physical Requirements
Able to drive a vehicle for long periods of time
Able to drive in extreme road/weather conditions
Able to walk, stand, kneel, bend, and climb
Able to lift light to heavy objects
Able to work in a stressful environment
Able to work in a multitude of conditions within a patient’s home
Able to assess and communicate with ill patients in an effective manner
Able to communicate with co-workers and the general public in an effective manner
Good speaking and hearing skills needed
Hours and days may vary depending on business needs and staffing requirement
Will work closely with others both inside and outside of the home
